As part of your commitment to the Chisholm Care programme, it is time to remind you about the importance of submitting your Chisholm Care Diaries by the due date. It is due by the end of Term 3 or the date specified by your PCA.

The Diary is not just a requirement and it is more than a record of hours spent. It is also a reflection into your service journey, the challenges you have overcome, the lessons you have learned, and most importantly, the positive changes you have contributed to your community.

Ensure your Diary entries follow the provided guidelines, this helps to maintain consistency and fairness when assessing everyone’s contributions. Please be sure to share your genuine thoughts, experiences and the ways in which service has impacted you.
