All students require parent/legal guardian consent (or decline) before they receive any vaccinations at school. This can be done through the link below which takes you to the online portal where your intent (consent or decline) will be registered: consentpublic (health.wa.gov.au).
Parents can also find information on the vaccines (as well as a hard copy of the consent form) by visiting this website: Adolescent immunisation (healthywa.wa.gov.au).
For those who prefer QR codes for information and/or consent (or decline), use the link below: 2026 Parent information and consent.
Alternatively, if parents would rather complete a paper version of the consent (or decline) form, please contact Mrs Kristi Astone at the College.
Parents are encouraged to complete this registration as soon as possible, to allow time for the Community Health Immunisation Team to process and prepare before their visit to Chisholm.
