This is a reminder that the Year 10 students have the opportunity to receive their Immunisation at Chisholm on Monday 26 May and Tuesday 27 May (Week 2.5), as per the School Based Immunisation Programme from WA Health.

Thank you to the parents/guardians who have already completed the form to consent (or decline) to vaccination at school. If you have not yet registered, parents/guardians of Year 10 students are asked to complete the form to consent (or decline) to vaccination at school via VaccinateWA. Click on the link below:

If parents prefer to complete a paper version of the consent form, please contact the College. Parents are encouraged to complete this registration as soon as possible, in order to allow time for the Vaccination Team to process and prepare before their visit to Chisholm.
