Parents/guardians of Year 7 and 10 students have received an email this week with information regarding the School Based Immunisation Programme.
Some parents are having difficulty with the link accessing the print copy of the Consent Form. A copy can be found in SEQTA Engage under School Documents.
Immunisations will be provided at Chisholm on the following dates:

Year 7 | HPV (human papilloma virus) dTpa (Diphtheria, Tetanus, Pertussis) | Monday 12 August and Tuesday 13 August |
Year 10 | Meningococcal ACWY vaccine | Monday 20 May and Tuesday 21 May |
All students require parent/guardian consent before they receive any vaccinations at school.
Parents/guardians of Year 7 and 10 students are asked to complete the form to consent (or decline) to vaccination at school via VaccinateWA. Click on the link below:

Parents are encouraged to complete this registration as soon as possible, in order to allow time for the Vaccination Team to process and prepare before their visit to Chisholm.
