Year 7 and 10 School-Based Immunisation Programme

The Department of Health offers a School-based Immunisation Programme that provides routine and recommended vaccines to WA high school students for free. Immunisations have already occurred for the Year 10s in Term 2 and this term will be delivered to the Year 7s at the College according to the below schedule:

HPV (human papilloma virus) 
dTpa (Diphtheria, Tetanus, Pertussis) 

Monday 17 August and Tuesday 18 August (Week 3.5)

Meningococcal ACWY vaccine 

Catch-Ups only from Term 2  
Tuesday 18 August (Week 3.5) 

All students require parent/legal guardian consent (or decline) before they receive any vaccinations at school. This can be done through the link below which takes you to the online portal where your intent (consent or decline) will be registered: consentpublic (health.wa.gov.au)

Alternatively, if parents would rather complete a paper version of the consent (or decline) form, please contact Mrs Kristi Astone at the College.

Thank you to the Year 7 parents who have already completed the registration and for those who have yet to consent/decline, please do so as soon as possible. This allows time for the Community Health Immunisation Team to process and prepare before their visit to Chisholm.
