Dear Parents/Guardians of Year 7 Students

This is a reminder that the Year 7 students have the opportunity to receive their Immunisation at Chisholm on Monday 18 August and Tuesday 19 August (Week 3.5), as per the School Based Immunisation Programme from WA Health.

Thank you to the parents/guardians who have already completed the form to consent (or decline) to vaccination at school. If you have not yet registered, parents/guardians of Year 7 students are asked to complete the form to consent (or decline) to vaccination at school via VaccinateWA. If parents/guardians prefer to complete a paper version of the consent form, contact Mrs Kristi Astone at  Student Services or by calling 9208 2500. Parents/guardians are encouraged to complete this registration by the end of this week, in order to allow time for the Vaccination Team to process and prepare before their visit to Chisholm.
