Dear Parents/Guardians of Year 7 and 10 Students,
Parents/guardians of Year 7 and 10 students have received an email this week with information regarding the School Based Immunisation Programme.
In 2025, the School Based Immunisation Programme will provide the following immunisations at Chisholm Catholic College on the following dates:

Year 7 | HPV (human papilloma virus) dTpa (Diphtheria, Tetanus, Pertussis) | Monday 18 August and Tuesday 19 August (Week 3.5) |
Year 10 | Meningococcal ACWY vaccine | Monday 26 May and Tuesday 27 May (Week 2.5) |
All students require parent/guardian consent before they receive any vaccinations at school.
Parents/guardians of Year 7 and 10 students are asked to complete the form to consent (or decline) to vaccination at school via VaccinateWA. Click on the link below:

Parents are encouraged to complete this registration as soon as possible, in order to allow time for the Vaccination Team to process and prepare before their visit to Chisholm.
